Part II: Prior Award & Lifetime Cap Attestation

Part V: Required Attachments Checklist

Official Transcript / Report Card: Registrar-sealed transcript demonstrating a cumulative GPA of 2.5 or higher (required for high school, undergraduate, and master's degree levels; incoming college freshmen must submit their final high school transcript). Note: Grade school applicants are exempt from submitting grades or transcripts.
